Description
- The student is not allowed to participate in any physical activity until a medical assessment is completed and information from the parent/guardian is provided to the school administrator/designate.
- The school administrator/designate informs staff who provide students with physical activity that the student must not participate in physical activity until their parents have returned the completed Fainting Episode Form.
- Once the completed form has been returned, the school administrator/designate informs the relevant staff (physical education teacher, coach, intramural supervisor) whether the student can participate in physical activity based on the information provided.
- The school administrator/designate files the Fainting Episode Form in the student’s Student Cumulative (CUM) File.
